A DAY IN THE LIFE AS A HOME HEALTH SUPERVISOR / REGISTERED NURSE (RN)


As a Home Health Supervisor, you are the initial contact for our patients and you provide them and their families with their start of care information. You design a plan of care that is tailor-made for each patient and revise it quarterly. You both deliver and supervise the delivery of skilled nursing services. You assist with the assigning of registered nurses (RN), licensed practical nurses (LPN), home health aides, personal care aides, or other nursing support staff to individual patients and perform in-home evaluations as required. You teach or supervise the teaching of household members who are responsible for assisting the patient with his/her nursing or personal care needs. You prepare and coordinate clinical and progress notes on nursing services delivered.


You call social workers and physicians to provide them with information regarding services offered by Care Advantage as well as rates of services to include in brochures. You follow up and complete start of care for referrals and other cases as assigned. You report progress of care to Social Workers who referred cases.


Under the direction and supervision of the Director of Clinical Services and the Senior Administrator, you work collaboratively with all departments to provide high-quality care in a safe environment. Your knowledge of OSHA and Virginia State Licensure regulations governing home and health and nursing practice assist you in training employees. You conduct orientation and training or supervise the orientation and training of agency employees assigned to the nursing service. You participate in quality improvement reviews, marketing, and recruitment.


You follow up and maintain relationships with clients through written and verbal communication. You keep the Home Health Director and Office Administrator abreast of all assigned patients' condition and/or changes in medical status. You report changes in the patient's medical or mental condition to the attending physician and to the agency employees and contractors involved in the patient's care. QUALIFICATIONS TO BE A HOME HEALTH SUPERVISOR / REGISTERED NURSE (RN)

  • At least 2 year of experience in a nursing role
  • Must possess an active Virginia nursing license
  • Ability to lift a minimum of 10 lbs
  • Ability to travel on a regular basis

WORK SCHEDULE
